Thank you for supporting the 2018 Philanthropy@Work campaign.

Pledges can be paid out over time by automatic bank withdrawal (ACH), credit/debit card, and payroll deduction (for eligible employees).  One-time gifts can be paid by credit/debit card, ACH, payroll deduction, check, or cash.

DONATION OPTIONS

  • Automatic Bank Withdrawal (ACH) (pledge or one-time gifts): Deducted directly from your bank account on the 20th of the month (or the next business day if the 20th falls on a weekend or holiday).

  • Credit card (pledge or one-time gifts): Processed to your credit card on the 10th of the month (or the next business day if the 10th falls on a weekend or holiday).

  • Payroll Deduction (pledge or one-time gift): Pledge payments will be deducted from your paycheck, for 26 pay periods (or longer if you so choose), starting the first pay period of the year.  A one-time gift will be deducted from your first paycheck of the year, unless otherwise noted.

  • PTO (pledge or one-time gift): One hour (or more) of your PTO will be deducted from each paycheck, for 26 pay periods (or longer if you so choose), starting the first pay period of the year.  A one-time gift will be deducted from your first paycheck of the year unless otherwise noted.  (For instance, if you are maxed out, hours can be taken from your next pay period.)

  • "Hour Club" (pledge or one-time gift): The equivalent of one hour (or more) of your pay will be deducted from each paycheck, for 26 pay periods (or longer if you so choose), starting the first pay period of the year.  A one-time gift will be deducted from your first paycheck of the year unless otherwise noted.

  • Cash or check (one-time gift only):  You may make a donation by cash or check.  Make checks payable to CHI Health Foundation, then give your check or cash and your completed pledge form (download below) to your local campaign champion or development officer.
